Pricing:

Price covers all sick, chronic care visits and an annual physical with your physician.

• 0-2 years: $65 per month without an adult member or $55 per month with an adult member

• 3-18 years: $45 per month without an adult member or $35 per month with an adult member

• 19-35 years: $60 per month

• 36-64 years: $75 per month

• 65+ years: $95 per month

• Homebound patients (any age): $150 per month

Discounts:

• Families: $225/month (2 parents and up to 3 children <18, then $20/child/month for additional children)

• Pay for the year and get a 5% discount

• Full time college students $300/semester

House Calls (subject to availability)

• 15 mins from office - $25/visit

• 30 mins from office - $50/visit

*Registration fee: one time registration fee or $75 per member or $150 per household
